Terms & Conditions:

(1)	If you fail to make full repayment of the statement balance specified in your credit card account statement on or before the payment due date, you shall pay finance charge on all outstanding balance on your credit card account pursuant to the relevant credit card cardmember agreement. You shall pay a finance charge on any monthly instalment amount due but unpaid. The finance charge is calculated by the prevailing interest rate applicable to your credit card account. For details, please refer to the Important Note on the credit card application form.
(2)	Market price was provided by the supplier on 1 July 2008 (including delivery service).
(3)	Interest Free "Cash Instalment" Plan is not applicable to full-time student applicants.
(4)	Principal Card Customers can enjoy perpetual annual fee waiver for one Hang Seng Credit Card only. Applicant who is currently holds or in the past 6 months has held any Personal Credit Card Principal Card issued by Hang Seng Bank Limited ("Hang Seng") will be entitled to one year full annual fee waiver only. All Supplementary Cards applied for at the same time will be entitled to the same annual fee waiver as is applicable for the relevant Principal Card. Supplementary Card application is not applicable to Full-time Student Principal Card Customers.
(5)	Customers may enquire the relevant merchants / associations for the latest offers before transaction. Hang Seng and the relevant merchants / associations will not be liable for any loss to customers due to changes of the offers. Hang Seng and each relevant merchant / association reserve the right to revise the terms and conditions of each corresponding offer from time to time. In case of any dispute, Hang Seng and each relevant merchant / association reserve the right of final decision.
(6)	Customers can have access to "City Chinese Restaurant" and "City Top" during off-peak hours only, except CityU staff.
(7)	Members of CityU Card customers can enjoy the rate of HKD28 per visit. You may also bring along a maximum of two guests at the same rate; or gain unlimited use by using the Swimming Pool Seasonal Pass that costs HKD600. Normal opening period of swimming pool is from April to November. Applicants of Swimming Pool Seasonal Pass should bring along a valid CityU Card for verification and photo copy upon application.
(8)	For details and terms and conditions of Platinum Card privileges, please refer to the booklet of "Year-round Privileges for Selected Hang Seng Credit Card" which will be mailed to customer separately upon card approval.
(9)	Customer is required to accumulate a spending of HKD500 or more with your Card during 11am to 12 midnight within the same day to enjoy the two-hour free parking offer.
(10)	Only applicable to full-time students of CityU. To enjoy the offer, students shall present valid student identity cards. After the fee waiver period, a Monthly Fee will be charged according to the fee schedule for holding the Hang Seng Integrated Account prevailing at that time.

	Information filled out in this credit card application form will also be used by CityU for the purpose of updating your personal information stored in the alumni database of CityU in connection with the alumni network development of CityU.
