Through this survey, Hang Seng Bank Limited (the ‘Bank’) hopes to better understand your needs and gather your opinions for future enhancement of Hang Seng MPF services. Please spare a few minutes to complete the questionnaire on or before 31 October 2025.

You may choose to return the completed questionnaire to us by mail or by fax (fax no. : (852) 3409 2108); or you may log on to Hang Seng Personal e-Banking (website: hangseng.com/e-Banking) and click the hyperlink under ‘MPF Overview’ in ‘MPF Services’ to complete and submit the questionnaire online.

Regardless of the channel for submission, the first 2,500 members who successfully submit the completed questionnaire will each get a HK$50 supermarket eVoucher.^Notes

You will not be entitled to the eVoucher if your HKSARG employee account has been cancelled or terminated by 31 October 2025 (i.e. deadline of the survey).

If you have more than one HKSARG employee account, please fill in one of your MPF membership numbers and submit the survey once only.

Notes:

• Each eligible awardee is entitled to a supermarket eVoucher (‘eVoucher’) once only, which will be distributed through SMS with a link to the awardees’ latest correspondence mobile phone number according to Hang Seng MPF record within 3 months after the deadline of the survey (Please note: SMS sender ID is #HASEsecure). Ineligible members will not be notified. Please fill in the ‘MPF Membership No.’ for our distribution of the eVoucher to eligible awardees in accordance with the relevant MPF record. If the MPF membership no. or the name provided is inconsistent with our MPF record or there is no valid correspondence mobile phone number according to our MPF record, we reserve the right to cancel your qualification without further notice.

  Please note that if there are any changes to your mobile phone number and/or name, kindly update your MPF records promptly to ensure that you can receive eVoucher and other important notifications.

• If the eVoucher cannot be distributed to the eligible awardees via the correspondence mobile phone number in our MPF record in January 2026, the eVoucher will be forfeited without further notice
• Members cannot choose or request to change the merchants supplying the eVoucher.
• For the use of the eVoucher, please refer to the terms and conditions in the eVoucher page. If the eligible awardees cannot redeem the eVoucher within the eVoucher Redemption Period, the eligible awardees shall be assumed to have forfeited the right to redeem the eVoucher. Any format of re-issuance, refund, exchange, compensation or exchange for cash will not be accepted. Once the right to redeem the eVoucher is forfeited, the Bank reserves the absolute rights to handle the eVoucher.
• The eVoucher cannot be cancelled, altered, transferred, refunded, exchanged for cash credit or other gift and benefits under any conditions once they are issued or distributed. Apart from the responsibilities stipulated by law, the Bank accepts no liability for any loss, damage or impact of the eligible awardees resulted from the eVoucher.
• The Bank is not the provider of the eVoucher and will not be responsible for any related obligations and liabilities. Any disputes arising from the eVoucher shall be resolved between the members and the relevant suppliers or providers directly.
• The Bank reserves the right to change these terms and conditions at any time and to determine whether a member is an eligible awardee. In case of any disputes, the decision of the Bank shall be final.
• The eVoucher shall not be construed as a recommendation, invitation, marketing or promotion of any goods, products or services.
