Through this survey, Hang Seng Bank Limited (the 'Bank') hopes to better understand your needs and gather your opinions for future enhancement of Hang Seng MPF services. Please spare a few minutes to complete the questionnaire on or before 31 October 2021.


You may choose to return the completed questionnaire to us by mail or by fax (fax no. : 3409 2108); or via the personnel registry of your department. Alternatively, you may log on to Hang Seng Personal e-Banking (website: and click the hyperlink under ‘Account Overview’ in ‘MPF Services’ to complete and submit the questionnaire online.


Regardless of the channel for submission, the first 2,500 members who successfully submit the completed questionnaire will each get a HK$50 supermarket cash voucher.Notes


The information provided in this survey is for survey analysis, opinions follow up by the Bank, and/or handling follow up action with the eligible awardees (if applicable).

You will not be entitled to the Cash Voucher if your HKSARG employee account has been cancelled or terminated by 31 October 2021 (i.e. deadline of the survey).

If you have more than one HKSARG employee account, please fill in one of your MPF membership numbers and submit the survey once only.



  • Each eligible awardee is entitled to a supermarket cash voucher (‘Cash Voucher’) once only, which will be sent by mail to the awardees’ latest correspondence address according to our MPF record starting from the end of December 2021. Ineligible members will not be notified. Please fill in the ‘MPF Membership No.’ for our mailing of the Cash Voucher to eligible awardees in accordance with the relevant record. If the MPF membership no. or the name provided is inconsistent with our MPF record or there is no valid correspondence address according to our MPF record, we reserve the right to cancel your qualification without further notice.

    Please be reminded to update your address and/or name by completing and returning the ‘Personal Details Change Form’ (HA91) if your correspondence address and/or your name are/is not updated in our MPF record. The HA91 form can be downloaded from the designated MPF forms webpage on Hang Seng Personal e-Banking or Hang Seng MPF website

  • If the Cash Voucher cannot be sent to the eligible awardees via the correspondence address in our MPF record in December 2021, the Cash Voucher will be forfeited without further notice.
  • The Cash Voucher cannot be exchanged, transferred, or converted into cash under any circumstances.
  • Members cannot choose or request to change the merchants supplying the Cash Voucher.
  • For the use of the Cash Voucher, please refer to the terms and conditions printed thereon. The Cash Voucher is not replaceable in the event of lost or damage or expiry of validity period after being collected by the members.
  • The Bank is not the provider of the Cash Voucher and will not be responsible for any related obligations and liabilities. Any disputes arising from the Cash Voucher shall be resolved between the members and the relevant suppliers or providers directly.
  • The Bank reserves the right to change these terms and conditions at any time and to determine whether a member is an eligible awardee. In case of any disputes, the decision of the Bank shall be final.
  • The Cash Voucher shall not be construed as a recommendation, invitation, marketing or promotion of any goods, products or services.
